Athletic Forms and Information
Athletic Clearance Process:
- All athletic clearance will now be done online at https://www.homecampus.com/login
- You will register on the website (email address needed). Link to registration tutorials (YouTube), if needed.
- Download and print the "Preparticipation Physical Evaluation" below or pick one up at the athletic office. This must be brought to the doctor for your physical exam. Physical exams are required each year.
- Both you and a parent will need to electronically sign the form on the athleticclearance.com website to get cleared.
The following items/information will be needed to get cleared online:

- Emergency contacts' phone numbers
- Medical insurance company name
- Medical insurance policy number
- Primary care physician name

- You must have medical insurance to play sports. If you do not have insurance, you can purchase insurance through Myers-Stevens Insurance. Forms are available in the athletic office or check their website.
