Athletic Forms and Information
Athletic Clearance Process:
- All athletic clearance will now be done online at https://www.homecampus.com/login
- You will register on the website (email address needed). Link to registration tutorials (YouTube), if needed.
- Download a "Preparticipation Physical Form" here or pick one up at the athletic office. This must be brought to the doctor for your physical exam. Physical exams are required each year.
- Both you and a parent will need to electronically sign the form on the athleticclearance.com website to get cleared.
- The following items/information will be needed to get cleared online:
- Emergency contacts
- Emergency contacts' phone numbers
- Medical insurance company name
- Medical insurance policy number
- Primary care physician name
- You must have medical insurance to play sports. If you do not have insurance, you can purchase insurance through Myers-Stevens Insurance. Forms are available in the athletic office or check their website.
